What: The Senior Health Center will hold an open house to introduce the media and community to this patient-centered medical home that provides fully integrated, comprehensive care to those ages 65 and older.
Guests can learn more about available primary care services and resources and meet Senior Health Center physicians and staff who will be available to answer questions and demonstrate how patients can optimally use the center’s offerings, which include internal medicine, care coordination with specialty physicians, nutritional and medication assessment and educational services.
FREE Flu Shots, FREE blood pressure screening, demonstration of VGo robotic monitoring system and light refreshments also provided.
